Electronic Disclosure System
The City of Atlanta Electronic Disclosure System is a web-based public disclosure system for ethics forms required under city law. It enables city officials and employees to file their city financial disclosure statements online and allows the public to review the information disclosed on the forms for actual and potential conflicts of interest.

Online Disclosure Forms
2009 Financial Disclosure Statement. Elected city officials, certain city employees, city board members, executive directors, and certain NPU officers are required to file a city financial disclosure form each year they serve and in the year after they leave city service disclosing sources of income and transactions with the City.
Conflict of Interest Disclosure Report. Used to publicly disclose a financial or personal interest in a decision, legislation, or matter pending before the individual or his or her agency.
Gift to City Report. Used to disclose gifts received on behalf of the City of Atlanta in the past year (1) from a prohibited source, (2) as a result of soliciting a donation, (3) as an honorarium, and (4) for addition to the general fund or the city’s inventory of property.
Travel Disclosure Report. Used to report reasonable hosting expenses paid by a non-city source for travel by a city official or employee in an official capacity.
